About this service
CHOICES is a Case Management Program for pregnant young mothers and families with pre school aged children.
CHOICES provides individual outreach in-home support, information and referrals to appropriate community agencies or health care providers.
The service provides intensive support designed to help develop pre birth education and parenting skills. CHOICES also offers assistance with issues concerning, housing, finance, domestic violence, drugs and alcohol, further education, employment, child care, recreation, emotional support and empowerment, and practical support such as transport to appointments. CHOICES aims to assist young parents to link in with community resources and strengthen extended family and friendship networks.
