About this service
Provides intensive outreach support to young people experiencing vulnerabilities through short-term brief intervention and longer-term comprehensive case management. Working with young people to plan and achieve goals in relation to housing, mental health, AOD misuse, justice/legal matters, employment/education and more. Offering more than just service coordination, this service plays an integral role in working with young people on an individual level to ensure that young people build positive and sustainable connections to self and communities of choice, have increased self-agency and decreased isolation.
